PPT

The School-level Planning and Placement Team determines whether a child is eligible for special education services and plans an Individualized Educational Program (IEP) for that child. It includes a school administrator, a teacher, a member of the Department of Special Services, the parents, and other appropriate staff. Parents may decide to include other individuals who can contribute to this process.

IEP

Under The Individuals with Disabilities Education Act (IDEA), an Individualized Education Program (IEP) is an annually reviewed written plan that describes the educational program for special education services and includes:

  • current levels of educational performance

  • planned special education and related services

  • annual educational goals

  • short-term instructional objectives

  • special transportation requirements

  • extent of participation in the regular school program

  • any necessary modifications of the regular program

  • amount of time of regular and special program

  • date on which the program will begin

  • criteria to determine if the goals are being achieved

  • exit criteria (i.e., when the special program is no longer appropriate)

504

Under Section 504 of the Rehabilitation Act of 1973, a 504 Plan is developed to ensure that children with identified disabilities attending elementary or secondary educational institution receive accommodations that will ensure their academic success and access to the learning environment.

Requirements:

  • have a physical or mental impairment that substantially limits one or more major life activities

  • or have a record of such an impairment

  • or be regarded as having such an impairment.

There is no standard 504 plan, usually the plan is a written document that generally includes:

  • Specific accommodations, supports, or services for the child

  • Names of who will provide each service

  • Name of the person responsible for ensuring the plan is implemented
